Kristen Warden is an accomplished healthcare operator and value-based care (VBC) executive with a proven track record of building and scaling multi-state organizations serving seniors. As President and Chief Operating Officer at Theoria, she spearheads daily execution and process optimization across the clinical and growth teams. Driven by a deep passion for the intersection of senior housing, primary care, and value-based models, Kristen is dedicated to positioning Theoria as the leading platform and preferred partner for senior housing operators.

Most recently, Kristen served as the COO of ConcertoCare, where she scaled in-home, risk-bearing primary care models in partnership with major health plans to serve more than 80,000 homebound seniors. Prior to that, she was an early leadership team member at Oak Street Health, an industry-leading network of primary care centers delivering value-based care to Medicare-eligible older adults. Earlier in her career, she managed a portfolio of assisted living communities for Enlivant and built her operational foundation at DaVita Dialysis.

Kristen holds a Bachelor of Science in Industrial Engineering from Georgia Tech and an MBA from the Kellogg School of Management. She resides in Atlanta, Georgia, with her husband, two children, and their corgi.
