Organizations don't struggle with improvement because people are ignorant or irrational. They struggle because their systems were designed for different goals. Promotion rewards confidence over curiosity. Planning rewards certainty over learning. Metrics reward activity over outcomes. Odds are you already know this. You've probably even said it out loud.
