During this panel session, we will describe the benefits of interoperability between the San Diego 211 Community Information Exchange and the San Diego Health Connect HIE. Through this effort, community information and patient consent will be shared from the CIE to the HIE. The HIE will enhance 211 data by augmenting with HIE data and returning to the CIE. The effort ensures that providers have more complete information to support their patients/members. The panel will discuss ways in which social needs data advances health equity and provides opportunities to identify populations affected by disparities. Actionable data allows providers/members to close gaps in care and fine-tune service offerings and to demonstrate opportunities for improved health equity through the availability of actionable community data.
