30 years building tools that add hours back to a clinician's day
CEO at RealActivity | AI Pioneer | BOSHUG Founder | Microsoft Keynoter

I've spent my career at the intersection of healthcare and technology, not talking about AI transformation, but actually building it. In 2001, I designed a symbolic AI triage engine that eliminated pre-op anesthesia visits and accelerated same-day surgery pathways.

In 2023, my team shipped RealActivity Physician Activity Copilot, the world's first AI harness focused on hospital administration. And 2024, we shipped Wheelhouse, a model fine-tuned on PeopleSoft healthcare operations data and executive prompt workflows.

What happened in between? I trained 3,000+ engineers across GE, IDX, and Microsoft in enterprise software patterns. I migrated 50+ HIPAA workloads to Azure. I built the CMS compliance and activity reporting platforms that health systems still rely on. I earned Microsoft MVP recognition and was named Azure Most Influential, not for evangelism, but for implementation.

Today I'm CEO & Chief AI Officer at RealActivity, a boutique Microsoft partner delivering mission-critical AI for academic medical centers and health systems across the Northeast. Our focus? Tracking system wide clinical time (cFTE), CMS complaint AI, and improving operational efficiency, the problems that keep CMOs and CMIOs up at night. I'm also the founder of OCS Labs, an AI lab building industry-specific AI for government, and sectors where getting it wrong isn't an option.

I founded BOSHUG, the Boston Healthcare Cloud & AI Community, which has grown into a global network serving 40k. I'm a Cloud Wars healthcare AI analyst, a top 3% rated global speaker (Microsoft Ignite, Cloud Wars, and beyond).

Global citizen based in Greater Boston. When I'm not building healthcare AI, you'll find me at a Sicilian bakery arguing about lemon cake, sailing, snowboarding, diving, or planning the next adventure. 114 countries, and counting.

I help teams modernize apps and platforms in the Microsoft ecosystem—Azure, GitHub, Power Platform, and identity (Microsoft Entra)—with a bias toward real-world constraints and practical outcomes. I’ve spent 20+ years building and advising solutions across manufacturing, healthcare, insurance, and financial services.

My sessions are story-driven and demo-heavy: less “cloud mythology,” more “here’s the pattern, here’s the pitfall, here’s the fix.” Recent topics include practical GPT/agent builds, Microsoft Fabric in real workloads, secure identity, and modern delivery pipelines.

Based in Milwaukee, available nationwide—and frequently in Europe (Nordics/Baltics, incl. Stockholm & Riga). I’m also the creator of the biweekly LinkedIn newsletter Tech Trekking, connecting emerging tech to what actually works in the field

I lead product and tech teams for several companies at once, on a fractional basis, mostly in healthcare. I work alongside the teams day to day: I listen, I make the calls, I move things forward one topic at a time, without breaking everything and without getting stuck in the fog. I simplify and I automate.

Biomedical engineer by training, 20+ years in product (PM, VP Product, CPTO, up to ExCom), in large groups and startups alike. I work at the intersection of large-scale digital infrastructure and public health — designing AI-driven systems that turn prediction into real-world care. Over nearly two decades, I've built consumer and healthcare platforms that operate reliably at national scale, from fulfillment serving hundreds of millions to AI for early disease detection in underserved communities.

My talks connect two worlds most people keep separate: the engineering discipline behind systems that can't fail, and the public-health opportunity in reaching people traditional systems miss. I speak on healthcare AI, last-mile and prescription delivery, reliable and observable systems, and safe AI agents at scale — grounded in production experience and peer-reviewed research, not slideware.

I'm an IEEE and Linux Foundation speaker, a peer reviewer for venues including AMIA, HICSS, and IEEE journals, and I mentor student teams and early-career engineers. My aim on stage is simple: leave the room with something they can build on.

Dr. Marlow Hernandez

Dr. Marlow Hernandez is a physician-executive with more than 15 years of experience in value-based care across the United States. He has led healthcare initiatives that combine clinical practice with structured system design to deliver measurable, life-saving outcomes for hundreds of thousands of patients. His work centers on improving access, strengthening coordination, and building scalable care models.

Hernandez holds a clear, consistent belief: “We do not turn patients away.” Guided by this principle, he has developed organizations that integrate primary care, specialty services, and advanced analytics to support proactive and personalized medicine. He applies data as a clinical tool, using it to identify risk early and improve long-term outcomes, especially for underserved populations.

Born in Cienfuegos, Cuba, and raised in the Miami area, he immigrated to the United States in 1993. During childhood, he witnessed gaps in healthcare while observing his grandmother manage complicated diabetes. At five years old, Marlow Hernandez promised he would become a physician and help her. That early promise shaped his broader mission to improve healthcare systems for entire communities. He earned a Bachelor’s in Neuroscience from the University of Miami and a Doctor of Osteopathic Medicine (DO) from Nova Southeastern University. He completed his training at Cleveland Clinic Florida and is a Diplomate of the American Board of Internal Medicine, a Diplomate of the American Board of Venous & Lymphatic Medicine, and a Fellow of the American College of Physicians (FACP).

With DO, MPH, and MBA training, he founded a healthcare organization that expanded from one clinic into a $3 billion revenue platform serving 400,000 members. Under his leadership, it became the fastest-growing primary care network in the U.S. for two consecutive years, according to Inc. magazine. During the COVID-19 pandemic, the organization achieved a sustained 60% reduction in patient mortality, findings published in the American Journal of Managed Care (AJMC). It maintained a Five-Star quality rating for five straight years. It grew from $0 to nearly $1.0 billion in revenue within two years under the inaugural CMS direct contracting entity (now ACO REACH).

In 2023, Dr. Marlow left the company, which reorganized as a private entity in June 2024 and continues to operate in Florida. He later co-founded a senior-focused healthcare organization in Nevada, including locations on Tropicana Ave in Las Vegas and in North Las Vegas. There, he introduced a 48-Hour Guarantee and supported the development of a proprietary platform that predicts 30-day hospitalization risk with over 90% accuracy.

He remains active in clinical practice with unrestricted licenses in Florida and Nevada. His service includes charitable medical care, disaster relief, frontline pandemic response, long-term faith-based giving, research publication, conference contributions, and civic recognition.
